Fillers For Acne Scars
Fillers for acne scars are an effective non-surgical treatment for selected types of atrophic acne scars, helping to restore lost volume, improve skin contour and create a smoother, more even complexion. While not every acne scar is suitable for filler treatment, carefully selected patients can achieve significant cosmetic improvement, either with dermal fillers alone or as part of a combination treatment plan.
Acne scars develop following inflammation caused by acne and are generally classified into three main subtypes: rolling scars, boxcar scars and ice pick scars. Each subtype has distinct characteristics and responds differently to treatment, which is why an accurate diagnosis by an experienced dermatologist is essential before deciding on the most appropriate management approach.
Rolling scars create an uneven skin surface, often described by patients as a series of “hills and valleys”. These scars frequently cast shadows under different lighting conditions and may be associated with underlying volume loss. In these cases, dermal fillers can restore lost support beneath the skin, reducing the depth of the scars and producing an immediate improvement in skin texture.
Boxcar scars have well-defined edges and a broad, flat base, typically measuring between 2 and 5 mm in diameter. In carefully selected patients, fillers can be placed beneath the base of these scars to gently elevate the depressed area, improving the overall appearance of the skin. Deep ice pick scars, however, usually respond better to procedures such as Punch Excision for Acne Scars or Subcision, depending on the individual scar characteristics.
Many patients present with a combination of different acne scar types across the cheeks, temples, forehead or chin. Because of this, treatment is rarely limited to a single procedure and often involves combining several evidence-based techniques to achieve the best possible outcome.
A wide range of treatments are available for acne scars, including Microneedling, chemical peels, Fractional Laser, Laser Resurfacing, Subcision and dermal fillers. The most effective treatment plan depends on the scar subtype, its severity, the degree of volume loss and the patient’s individual skin type and expectations. In many cases, combining fillers with other collagen-stimulating treatments provides superior long-term results compared with any single treatment alone.
Several different filler products are available for acne scar treatment. Temporary fillers are most commonly based on hyaluronic acid, while bio-stimulatory fillers such as Poly-L-Lactic Acid (commercially available as Sculptra™) stimulate the body’s own collagen production over time. The most appropriate product is selected after a detailed consultation, taking into account the location of the scars, the desired outcome and the advantages and limitations of each filler type.
